JOB REQUIREMENTS: Position Description: The Patient Coordinator inComprehensive Pain and Spine serves as the first and continuous point ofcontact for patients to guide them through health care processes.Assumes responsibility and accountability for intake of all new andreturning patients in the Department of Neurosurgery and SpineCareClinic. Manages and schedules referrals for the Spine Service Line.Creates a seamless model of access and coordination for all patients.Identifies barriers to treatment and provides individualized assistanceto facilitate timely access to alternate services where appropriate.Primary Functions Applies developed workflows to coordinate efficientlyand effectively the intake, triage, scheduling and navigation of carefor new and returning patients. Triages a high volume of incoming phonecalls, responds to initial requests for service and obtains accurate anddetailed information for each patient including prior medical history,insurance status, outside records, films, and reports. Documentsinformation accurately, thoroughly, and appropriately in the medicalrecord. Applies comprehensive understanding of Spine Care program,systems, and physicians\' specialties to align scheduling practices withpatient centered scheduling initiatives, ensuring patients are scheduledwith the most appropriate provider(s) the first time. Determines theappropriate level and need for service, and prioritizes urgent orexpedited services for new patients when applicable using complexalgorithms and-when necessary-collaboration with faculty, referringphysicians, nurses and clinic management. Communicates and coordinatesinitial treatment plans with patients, patient families, and healthcareprofessionals via phone and/or written correspondence. Coordinates andschedules program and provider visits; communicates appointments topatients and provides customized written and verbal instructions.Explains nature of treatment offered and identifies individuals\' needs;links patients to alternative resources and health care personnel asappropriate. Employs excellent customer service standards whileinteracting with patients, faculty, colleagues and representatives fromoutside facilities. Communicates with outside facilities to request andfollow up on records needed; transfers data from one program to anotherto ensure information is available to providers. Provides assistance,training, and/or coverage to other team members as needed; acts asvaluable resource for all clinic staff.
