　　Barton Orleans Primary Care:

　　Joining our dynamic team, you will work alongside a supportive group ofexperienced providers, nurses, and support staff in our team-basedclinic while developing and managing your own patient panel. NorthCountry Primary Care Barton/Orleans is one of two primary care practicesat North Country Hospital, a critical access hospital that is recognizedby NCQA as a Patient-Centered Medical Home and is also a certified RuralHealth Center.

　　North Country Pediatrics

　　This is a full-time outpatient pediatric position in a four-providerhospital owned pediatric practice (3 MD, 1 APP) with excellent physiciansupport and mentoring. The candidate will see outpatients for well care,acute care and chronic care maintenance 4 days per week and one Saturdaymorning per month and will cover first call (outpatient only) withphysician backup 2 nights per week.

　　North Country Pediatrics is a RHC certified Medical Home that offerswrap around care to our patients including WIC in the Medical Home,dietician, asthma trainer, DULCE worker, case manager and embeddedtherapist.

　　We are looking for a well-trained APP, ideally with pediatricexperience, who works well in a team, is dedicated to high qualitypatient care and looking to grow with the position as we participate ina lot of QI projects
