　　The Advanced Practice Nurse APN practices in collaboration with the outpatient department s behavioral health physicians, nurses, clinicians and other health care professionals,

　　The APN provides behavioral health care services through assessment, diagnosis, treatment, clinical judgment and management to restore, maintain and improve the health status of patients,

　　Job functions include:

　　1,Provides psychiatric evaluations on all new patients and assesses for appropriate level of care,

　　2,

　　Active member of the Partial Hospital and outpatient team, attending weekly meetings, facilitating education groups, and coordinating with team and other providers as indicated,

　　3,

　　initiates psycho-pharmacological interventions when appropriate,

　　4,

　　Provides medication management as programs require,

　　5,Coordinates with other medication assisted providers to ensure patient safety

　　6,Administers medication such as long-acting injections when requested

　　7,Responds to crisis situations by directing others in the fulfillment of their responsibilities and by directly interacting with the patient in crisis,

　　8Works Collaboratively with team members to support patient care, treatment, and outcomes,

　　9Acts as a resource person regarding patient care in specialty area,

　　10,Organizes, supports and actively participates in maintaining a psychiatric milieu program,

　　11,Uses the nursing process to correctly complete and/or document relevant patient assessment and care information,

　　12,Notifies the assigned physician of pertinent patient data related to medication use, abnormal, laboratory results, and of significant alterations in patient status and behavior,

　　13,

　　Collaborates with and assists other members of the health team in interpreting to the patient and/or his/her family, significant information related to the patient s present and related health needs,

　　15,Provides education about available services and fosters positive attitude toward department,

　　16,Develops and implements teaching plans for patients and/or their families, as needed, and in collaboration with other health team members, which are based on a comprehensive assessment of patient problems,
