Position Overview:
The Medical Social Worker develops and implements a plan for home health/hospice social work services which includes effective psychosocial interventions for patients as well as education of family/significant others.

Master's: social work (MSW)
CERTIFICATION & LICENSURE
BLS-Basic Life Support Healthcare Provider
OR LCSW-Licensed Clinical Social Worker
DL-California Drivers Class C
AUTO-Automobile Insurance
PREFERRED EXPERIENCE AS TYPICALLY ACQUIRED IN:
Title XXII and Medicare Conditions of Participation require one year of experience as an MSW in a healthcare setting.
Experience working with patients/families dealing with life-threatening illness, death, and grief.
Recent home health and hospice experience preferred.
SKILLS AND KNOWLEDGE
Ability to recognize people's emotional, psychosocial, economic, and spiritual needs and concerns during difficult times.
Knowledge of community resources.
Knowledge of inter-professional relationships in the implementation of patient services and programs.
Knowledge of death, dying, grief, and bereavement processes.
Strong interpersonal relationship skills.
Strong organizational, written, and oral communication skills.
Ability to maintain positive relationships in dealing with a multidisciplinary team and community resources.
Ability to organize and use time efficiently.
Ability to be open and flexible.
Possess a current driver’s license in CA and access to a reliable automobile.
Proof of liability insurance for the car used during working hours.

Pay Range is $49.91 to $61.74 / hour
The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, department equity, training and organizational needs.
