  Facility:Dayton Children's - Main CampusDepartment:Social WorkSchedule:Full timeHours:40Job Details:The Medical Social Worker is responsible for working in a team structure with the RN Case Manager and other Integrated Care department staff, to effect overall care coordination for the transition of care and complex discharge planning process. Through psychosocial assessment and intervention strategies, is responsible for assisting patients and their families in understanding and learning to cope with illness and disability. Assists families in resolving sophisticated personal and environmental difficulties which interfere with effective social functioning and acquiring maximum benefit from medical care. Participates as a professional team member in patient care planning along with the hospital and community agency personnel. Provides education and consultation to health care professionals and the community regarding the impact of psychosocial factors. Responsible for the delivery of social work services which are available to the patient, the patient’s family, and other persons significant to the patient. This position will collaborate interdepartmentally with all divisions of the hospital, inpatient, ambulatory, and Emergency. Social Workers assume additional leadership duties to help with onboarding, training, coaching and job shadowing for newer staff, participate in the department’s peer mentoring program for new staff around developing social work skills and completed additional special projects as requested. LISW Social Workers are eligible to volunteer work time to provide group supervision for independent licensure preparation for dependent licensed social workers and students, as well as attend other leadership opportunities as able. Social Worker participates in the social work coverage plan and the holiday rotation coverage plan.

  The Child Advocacy Medical Social Worker is responsible for serving patients in the emergency department, inpatient units, and the Child Advocacy Clinics when there is a concern for suspected child physical or sexual abuse or child neglect. As an integral part of the Child Advocacy team, the social worker assists families through assessment, collaboration, coordination, mandated referrals, education, community resource linkage and advocacy. This position provides psychosocial assessment of the patient and family, including child abuse and neglect risk factors, protective factors, resources, safety, and other needs and barriers. Collaborates with Child Advocacy Chief, referring physicians, nurse practitioner, medical providers and team, along with community agencies including Children Services and Law Enforcement. The Child Advocacy Social Workers are involved in all the child abuse cases of the department for follow up, photo documentation, court testimony, investigatory huddles and conferences, communication and coordination with investigators from Children Services agencies across the region and all law enforcement agencies in multiple jurisdictions, in addition to three Child Advocacy Centers in Montgomery County (Care House), Greene County (Michael’s House), Warren County Child Advocacy Center, and attending the Multidisciplinary Team Meetings of the Clark County Child Advocacy Center. Valid driver's license required for services at Child Advocacy Center clinics and community education events.
