We currently have an exciting full-time career opportunity available to join the HKS team as a Medical Coordinator/ Direct Support Professional in our Wantagh Location.
The Medical Coordinator/DSP will assist the residence manager and the RN in the delivery and documentation of nursing services, in addition to carrying out traditional DSP activities, to residents of the Wantagh IRA.
Essential Duties:
Administration of and recording of medication prescribed to be given within the program, as needed.
Maintain the MAR and ensure the ordering of all medications, under the direction of the RN
Monitoring consumers’ vitals, including weight/blood pressure, as needed and reporting any variations to the RN for follow-up. Monthly weights
Participate at meetings, committees and in-services, as needed.
Provide assistance with the toileting needs of specified individuals, as needed.
Complete daily documentation in the Nurse’s log of interventions as needed.
Oversee mealtime routines and protocols.
Schedule medical appointments accompany residents to the appointments and ensure follow-up of recommendations.
Participate in pharmacy review, as needed or requested by the RN
Monitor diets and menus as provided by the dietician.
Supervise and positively interact with the consumers following all residential habilitation plans.
Provide consumers with community integration opportunities.
Direct consumers in their personal routine; give consumers positive reinforcement as appropriate.
Actively listens, builds trust and positive relationships, communicates effectively to direct consumers in their personal routine, and ensures positive reinforcement as appropriate.
Carry out residential habilitation plans and collect data.
Promotes activities in the community.
Provide first aid treatment as necessary.
Prepare meals utilizing the provided menus.
Maintain the order, safety, and cleanliness of the residence.
Transport consumers to day programs and appointments as necessary
Interact with parents and professionals from outside Agencies.
To complete all other assignments as indicated by your supervisor and/or the direction of the RN.
Required Education
High School Diploma or GED equivalent
Preferred Experience
Experience working with the developmentally disabled.
Knowledge of OPWDD regulations
License/Certification:
Valid NYS Driver’s license (Required)*
