Shift:Monday-Friday: 8:30am-5:00pm100% OnsiteRequirements:* COVID-19 Vaccination* Medical assistant or pharmacy background* Ability to Multi Task-work off of 3 monitorsMain Responsibilities* Working with Referral sources to gather discharge information notes/ to educate/setup patient.* Working within 5 different portals--technical* High sense or urgency* Must keep an eye out for new referrals coming in---follow up on existing and new.* Knowledge of Medicare guidelines for patient services/ requirements-big plus!* Referrals coming in can be for patients being discharged from Hospital same day or tomorrow.* Data entry for referral* No verification of benefits ---only gathering info for coordination of benefit team to process.* Listing for patients is setup and split Alpha* Go through a documentation checklist of what is needed to send to benefits team for next steps.* Responsible for securing "Nursing Agencies" for additional services needed.* Must be able to read a patient chart--and navigate through to pull up the information.* Onboarding a patient-permissions and coordinating signatures.* Informing patient financial obligation---calculator is provided for them to use.Beacon Hill is an Equal Opportunity Employer that values the strength diversity brings to the workplace.
