　　Job Summary and Qualifications

　　The Licensed Practical Nurse (LPN) performs all treatments and procedures as outlined in the Standards Manual and applicable regulatory and professional entities' guidelines, standards, protocols and procedures within the scope of LPN practice. With RN supervision, you will support the nursing process as follows:

　　You will collect health information per interviewing, observing, and measuring vital signs and specimen collections.

　　You will monitor patients’ safety, symptoms, changes in behavior and/or condition, IV sites, and medication side effects and effectiveness

　　You will report unsuccessful interventions and communicate patient’s needs to RN, physician, social services, Physical/Occupational therapy, and Care Management staff.

　　You will make suggestions to improve and individualize care. You will administer specified medication accurately and in accordance with hospital and regulatory agencies’ policies and legal limitations.

　　You will assist patients with Activities of Daily Life

　　You will provide information to patients and their families regarding the unit, medications, procedures, and discharge plans.Qualifications:

　　This position requires a level of knowledge which is acquired through education preparation in an accredited LPN program.

　　Current NH LPN license or compact state.

　　Working knowledge of underlying applicable principles, policies and procedures for the following entities: CDC, CMS, ANA, NH State Board of Nursing (Nurse Practice Act)

　　IV Therapy Certification required.
