The Licensed Nurse is responsible for planning, organizing, directing, and evaluating nursing services as part of the Interdisciplinary Team. In collaboration with the Interdisciplinary Team, administers an individualized, comprehensive and Resident centered Plan of Care in accordance with current applicable Federal, State, and local standards, guidelines and regulations as may be directed by the Neighborhood Coordinator, Administrator, Director of Nursing, Primary Care Nurse or Resident.

Required Qualifications:

Washington Licensed Practical Nurse upon hire.

National Provider BLS - American Heart Association upon hire.

5 CENTER PRIMARY NURSE: Teaching Experience.

Preferred Qualifications:

One (1) year LTC geriatric experience preferred

Experience as a staff nurse with LTC

Supervisory experience with nursing assistants

Experience with an interdisciplinary team

5 CENTER PRIMARY NURSE: Three to five years direct care experience; MDS/RAI Experience

About the Team

Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.

HCC provides a full range of services and support for people of all ages, especially those facing chronic or life-limiting illnesses.
