Homecare Scheduler Job Location: Norwich, Connecticut

　　Homecare Scheduler

　　Masonicare Home Health & Hospice - Norwich, CT

　　Day Shift / 40hrs/wk

　　Summary of Position: To prepare, coordinate and maintain the day-to-day activities related to scheduling HHA visiting staff by utilizing them to address patient primary needs. Perform clerical duties related to the scheduling department function. Ensure that agency goals and objectives are met.

　　Essential Duties and Responsibilities:

　　Input schedules into the computer scheduling system under the direction of the Supervisor. Schedules visits based on continuity of care for both staff and patients. Coordinates scheduling adjustments needed on a daily basis while monitoring and reporting assigned visits not completed.

　　Communicates with patients regarding visiting staff schedules and schedule changes as needed.

　　Builds associated personnel into patient files and reviews schedules to assure maximum consistency of visit staff for each patient scheduled. Assigns visits based on maintaining a low re-hospitalization rate.

　　Matches schedules to ensure that all visits are completed in a timely manner by the staff.

　　Performs functions to support the clinical verification process as needed

　　Communicates effectively and tactfully with patients and/or their families, recognizing their age, cultural diversity, needs, abilities and physical condition.

　　Performs other duties as assigned.

　　Minimum Qualifications:

　　Education: High School Diploma or equivalency required. Some college preferred.

　　Experience: 2-3 years related work experience, scheduling of field staff preferred.

　　Demonstrated working knowledge of computerized scheduling software.Certificates, Licenses, Registrations:

　　Other: Must possess reliable transportation and a valid Driver’s License
