　　Position Purpose:

　　Help your patients stay in the comfort of their own homes by providing compassionate care as a Home Health RN.

　　What you will Do:

　　Manage your caseload of patients throughout the Cedar Valley

　　Assess, plan, implement and evaluate care of patients.

　　Supervises and coordinates care provided by LPNs and PCAs.

　　Facilitates communication with physicians, families and other team members.

　　Hours/Schedule:

　　.8 fte, Days. Evening/night and weekend call rotation.

　　Minimum Qualifications:

　　Active IA RN license

　　BSN preferred

　　Ideal candidates wilk have at least 2 years of experience
