About Home Care Planner -MSW Graduate

　　Job description:

　　· Screen patient charts with presenting ADL deficiencies.

　　· Meet and greet patients in the waiting room.

　　· Medicaid and Medicare Planning

　　· In Person Assistor with New York State of Health.

　　· Home Care Planning.

　　· DME referrals.

　　· Plan Of Care’s

　　· Perform other duties as needed or requested

　　Knowledge, Skills And Requirements:

　　Master's degree in Social Work (MSW) required; human services experience in the field of aging preferred.

　　Bi-lingual Creole, or Spanish.

　　NYS license and registration to practice as a Licensed Master Social Worker in New York State preferred.

　　Able to perform data entry.

　　Must be computer literate with basic skills in Microsoft Word and Excel.
