SummaryDescription: As a care partner, the Home Care Assistant is responsiblefor assisting with a variety of personal care services and homemakertasks that support and enhances the person\'s desire to live at homesafely with dignity, independence and choice The services provided willoptimize the level of physical, emotional, psychological, social andspiritual wellness or wellbeing. The Home Care Assistant and Companionwork independently under the direction of the home care director withclinical support from the community\'s Nurse. Must maintain a caringattitude towards residents at all times and be capable of good judgmentin all situations. Essential Functions: Performs job functions in amanner consistent with the mission and goals of The Loomis Communities.Adheres to The Loomis Communities Compliance and Ethics Program.Demonstrates an awareness of, a respect for and attention to thediversity of the people with whom they interact (persons served,personnel, families/caregivers, and other stakeholders) that isreflected in attitudes, behavior and services. Interacts with residentsby providing direct personal care, medication reminders, escorts,transportation, shopping and supportive homemaker services for residentsto promote independence and choice. Maximizes residents\' overallwellbeing through knowledge of residents\' interests and using thatknowledge to encourage residents to continue engaging in pursuits thatfoster continued wellness. Attends to the wellbeing of residents andincludes residents in decision making regarding their care in accordancewith a personalized plan of care/service plan. Supports residents indaily living decision making regarding care and services in accordancewith established service plans. Maintains and completes requireddocumentation. Provides clear, timely communication related to changesin health, emotion and behaviors to direct supervisor. Meetseducational, training and competency requirements on time includingSAMMS. Responds to emergency situations in a prompt and calm manner andprovides basic first aid care when needed within the limits of trainingand the policies of The Loomis Communities. Required Education: HighSchool Graduate Preferred Education: Nursing or other health relatedundergraduate programs Required Experience: Certified Nursing Assistantor Certified Home Health Aide Preferred Experience: Six months previousexperience working with some type of long term care client, CertifiedNursing Assistant, Certified Home Health Aide License: MassachusettsDriver\'s License Certified Nursing Assistant CPR Certified
