Reporting to the High Value Care Pharmacist, this position is responsible for utilizing medication access expertise to plan, coordinate, and prioritize patient care activities considering patients’ unique medication needs and desired outcomes in collaboration with the Practice Innovation team and patient’s primary care team, optimizing patient outcomes and ultimately performance in value-based payment models. This position will support Practice Innovation efforts to redesign medication-related activities that positively impact quality of care delivered, enhance the experience of care for clinicians and patients, and optimize return on value-based opportunities.
PRINCIPAL DUTIES AND RESPONSIBILITIES :
Provides pharmacy services in a specialty area under the direct supervision of a licensed pharmacist/licensed independent practitioner while demonstrating advanced technical skills and ownership for the work area. Responsibilities include:
Applies professional knowledge while working in the specialized area of the pharmacy.
Supports and enhances access to affordable and quality care in ambulatory practice areas.
Triages and escalates any questions or issues outside the pharmacy technician scope of practice to the staff pharmacist or supervisor as needed.
Manages patients, as assigned by the High Value team under PI, with responsibility for contributing to improvement of performance on medication-related value-based contractual priorities. Examples of activities include navigating barriers to medication access; reviewing and monitoring medication-related patient outcomes; proactively identifying, prioritizing, and monitoring medication-related action items; and assisting with retrospective chart reviews.
Facilitates communication between patients/families and the healthcare team, documenting and clearly communicating identified problems, risks and health concerns with other members of the health care team.
Works in conjunction with the PI team, UPG, the Health System, and external payors to facilitate medication-related patient care throughout the health care system.
Uses appropriate systems for processing of patient information, including documentation of activities within Electronic Medical Recordkeeping (EMR) system.
Engages in periodic review of data reflecting performance and outcomes.
Represents Practice Innovation on various UPG and UVA committees and participates in meetings, as requested.
Actively participates in departmental projects/operations including identification, implementation, execution, and monitoring of quality improvement (QI) activities that support PI priorities and the organization’s mission, vision, financial and strategic plan with an emphasis on medication-related contracted priorities. Examples of QI initiatives include: closure of care gaps, improving clinical outcome measures, and optimizing workflows for capture of relevant clinical information.
Continually seeks out additional training opportunities and stays abreast of new and changing information for these assigned areas.
Supports the training of new employees and participates in the creation and maintenance of training materials.
Supports educational mission through training of pharmacy residents, students, and pharmacy technician trainees and continuous professional growth.
Demonstrates advanced technical skills and ownership for the work area.
Ability to work independently in work area with little oversight.
Follows policies and procedures, including those of UPG, UVA Medical Center, CMS and other contracted payers with which the entity may contract.
In addition to the above job responsibilities, other duties may be assigned.
REQUIRED QUALIFICATIONS (Knowledge, Skills & Abilities) :
Education:
High School Graduate or Equivalent.Level and type of experience REQUIRED:
24 months of relevant pharmacy technician experience required
Licensure: Commonwealth of Virginia Pharmacy Technician Registration required.
2+ years credentialed as a CPhT through National Healthcareer Association (NHA) or Pharmacy Technician Certification Board (PTCB).
Preferred Experience:
Experience in pharmacy access, utilization review, and/or Managed Care preferred.
Experience with EPIC preferred.
Skills:

Excellent interpersonal, tact, and diplomacy skills.
Proficiency with Microsoft Office Suite (Word, Excel, PowerPoint).
Strong track record of problem and issue resolution.
Ability to track and communicate aggregated results of interventions.
Preferred:
Familiarity with internal and external sources of demographic, financial, and utilization data.
Physical Demands:
Proficient communicative, auditory and visual skills; Attention to detail; Computer Keyboard; Repetitive motion: typing; This is primarily a sedentary job involving extensive use of desktop computers and sitting for prolonged periods; Ability to lift/push/pull less than 20lbs.SUPERVISORY RESPONSIBILITIES : The incumbent has no positions reporting to him/her. However, clinical departments, outreach practices and administrative staff collaborate closely with the incumbent.
