  Job Description:

  The Clinical Program Coordinator assesses, plans, implements and coordinates treatment processes from diagnosis through follow up, specifically guides patients through the collaborative process with all medical care providers and ancillary support programs and provides educational support along the continuum.

  Job Essentials

  Provides patients with information regarding available resources and options to facilitate shared decision making with their care providers. Provides hands on clinical care within professional Scope of Practice. Organizes and analyzes data to develop an individualized, comprehensive plan of care and follow up. Works with utilization review and health plans and other insurance companies to assist patients with insurance questions. Collaborates with all disciplines in forming a timely and appropriate plan of care and links with clinical trial programs for information and clinical trial participation.

  Provides patient and community education. Links with community/outside agencies as an educator and resource. Demonstrates good presentation skills and strong communication skills including interpersonal communication, warmth and sensitivity to treatment. Assists with the development, implementation and operation of patient education and facilitation of follow up care. Assists in the development and implementation of quality improvement activities to include patient satisfaction monitors and quality of life monitors. Monitors and evaluates trends and changes in clinical, service, and cost outcomes to various interventions. As applicable, responsible for maintaining certification of program.

  Acts as a resource, role model and mentor for the professional staff within the specialty area. Acts responsibly in conserving resources and collaborating with department projects and programs. Utilizes research to improve nursing practice and to stay abreast of national and regional standards, diagnosis and treatment. Provides clinical expertise in the specialty area and is a pathfinder in the identification and development of quality patient care delivery.

  Engages with physicians at the service line level to achieve clinical, operational and service goals as appropriate. Acts as a resource and primary contact for physicians in the department. Enhances physician productivity by proactively identifying and removing operational barriers and responds to physician's operational needs.

  Minimum Qualifications

  Current RN licensure in the state of practice.

  and -Basic Life Support Certification (BLS) for healthcare providers.

  and -Unit specific certification as required (i.e., ACLS, PALS, etc.).

  and -RNs hired or promoted into this role need to have or obtain their BSN within three years of hire or promotion.

  Two years of clinical nursing experience.

  and -Experience in assigned clinical area.

  and -Leadership and mentoring experience.

  Preferred Qualifications

  Bachelor's degree in Nursing (BSN). Education must be obtained from an accredited institution. Degree will be verified.

  Physical Requirements:

  Ongoing need for employee to see and read information, labels, monitors, identify equipment and supplies, and be able to assess patient needs.

  and -Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.

  and -Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, etc.

  The hourly range for this position is listed below. Actual hourly rate dependent upon experience.

  $38.83 - $57.46
