　　Healthcare Coordinator / Nurse: Seeking a Healthcare Coordinator who is responsible for the overall management of health and well-being of the residents within the assisted living community serving both assisted living and dementia residents.  The responsibilities include, but are not limited to, all clinical programs, hiring supervising and training of medication technicians and/ or wellness nurse (s); clinical quality assurance and regulatory compliance.  The Healthcare Coordinator is also responsible to be knowledgeable in Alzheimer's / dementia, the Greenfield mission, best practices, and state regulations.

　　Qualifications:

　　Maintains a current state license

　　Professional Licensed Nurse [LPN] or Registered Nurse [RN]

　　Minimum of one (2) year of experience in assisted living or long-term care

　　Experience with a dementia population 

　　Nursing management experience preferred

　　Exhibits a caring heart with a passion to care for an elderly population

　　Good clinical assessment skills

　　Experience using healthcare technology

　　Good organizational and time management

　　Able to work flex hours and be available to respond to community during off-duty hours regarding clinical concerns and emergencies

　　Possesses good judgement, problem solving, and decision-making skills

　　Possesses the ability to effectively read, write and communicate
