　　Responsibilities:

　　Work with consumers in assessing goals and implementing a care plan.

　　Manage chronic behavioral health, medical and substance abuse conditions.

　　Link consumers to health and community resources.

　　Work collaboratively with health providers and other community service providers.

　　Promote primary and preventative care.

　　Assist consumers in reducing or eliminating the need for hospital/ER usage.

　　Manage all aspects of consumers electronic health records.

　　Promote a culture of inclusion and belonging.

　　Requirements:

　　Bachelor or Master Degree in Human Services or similar field with at least two years' experience working with individuals living with mental illness, substance abuse or other disabilities preferred. Proficient with Microsoft Office and other web-based programs. Prior experience with Medicaid delivery system preferred but not required. Must have reliable transportation. Valid New York State Driver License. Must have the ability to work in a fast-paced environment.
