JOB REQUIREMENTS: This position will primarily serve clients inMilwaukee County. The Guardianship Representative is responsible forproviding informed substitute decision-making on behalf of the wards ontheir caseload as it relates to their medical care, placement and socialsupport services. The Representative advocates for the needs, desiresand rights of each ward ensuring the greatest level of independence,while balancing the personal choices and best interest of theindividuals. ESSENTIAL DUTIES (FUNCTIONS) include the following: *Safeguards the health as well as the physical, mental, and financialwell-being of wards by making informed, appropriate, and sound decisionsregarding their care. * Establishes and maintains positive workingrelationships with service providers, social service agencies, fundingsources, family members, court employees, etc. by ensuring timelyresponses to inquiries while overseeing the care and safety of thewards. * Coordinates service authorizations with the ongoing caremanager, monitors the appropriateness and thoroughness of wards supportservices, and provides consents for the provision of services. *Participates in and provides direction for planning activities relatedto ward care, which includes advocating for services, meeting theward\'s best interest, taking into consideration the safety and personalpreferences. * Acts as liaison with family members, wards, and otheragencies in difficult situations by communicating and mediatingdiffering perspectives as it relates to the wards\' care and services.* Responses to emergency situations requiring decisions related tocare, necessary notifications and final arrangements. * Conductsroutine visits to monitor the ward\'s care and functioning in theirresidences,...
