　　The CSWA Social Worker, under direct and continuous supervision, provides comprehensive biopsychosocial/spiritual assessment, diagnosis and/or treatment/interventions/advocacy of patients and their support systems.

　　Required qualifications:

　　Master's Degree Social WorkerPreferred qualifications:

　　Upon hire: Social Worker License

　　Upon hire: Certified Oncology Social Worker (OSW-C) or obtain within 3 years of hire.

　　Upon hire: Advanced Certified Hospice & Palliative Social Worker (ACHP-SW) or obtain in 2 years of hire.

　　Healthcare experience.

　　Home health/hospice or clinic/ambulatory care experience preferred.
