The Community Resource Representativeis responsiblefor building new relationships and maintaining relationships withphysicians, clinic and physician staff, hospital Case Managers,schoolsand the community. This position has a responsibility to positivelyimpact Agency growth and referral source satisfaction through acommitment to excellent customer service Serves as a resource/point ofcontact for referral sources by conducting one-on-one visits to buildand maintain relationships that result in census growth. Provides weeklyreports on contacts generated (i.e., physician, hospital Case Manager,key office staff, etc). Sets up programs yearly to enhance referralrelationships. Maintains a current knowledge base of all the servicesprovided by Horizon Home Care & Hospice. Assists the Director ofMarketing and Sales with: The annual marketing/sales plan andparticipating in health fairs, job fairs, exhibitions, etc. Supports anddemonstrates a customer service commitment to internal and externalcustomers through adherence to Horizon Home Care & Hospice customerservice standards and processes. Participates salesmeeting andprogram/team meetings when invited. Ensures conflict resolution withreferral sources, Agency staff and patients. Meets with patient\'s andfamilies to consult regarding services provided by Horizon. Meets goalsas established through the Agency\'s planning/budgeting process.
