Position Summary

As a Community Health Worker at MSU Health Care Inc., your role will be vital in connecting individuals with the healthcare services they need and empowering them to take control of their health through:
Conducting assessments of individuals' health and social needs and providing health education and guidance on preventive care and healthy lifestyle choices.
Advocating for patients within the healthcare system, ensuring they receive appropriate care and assistance with navigating the healthcare system.
Connecting patients and community members to local resources, support services, and social programs that can address their specific needs.
Developing and implementing community-based health promotion programs and initiatives to address prevalent health issues and improve community well-being.
Recognizing and respecting the cultural, linguistic, and socio-economic diversity of the community and tailoring services accordingly.
Maintaining accurate and up-to-date records of interactions with patients and community members and providing reports as required.
Collaborating with healthcare providers, social service agencies, and other community organizations to ensure coordinated care and support.
Engaging in community outreach activities to identify individuals in need of healthcare services and promoting MSU Health Care Inc.'s programs and resources.
This is an off-dated position funded for 9 months from date of hire with possible extension contingent upon funding renewal.
Unit Specific Education/Experience/Skills
Knowledge normally acquired from specialized training such as that acquired in two to three years of college in healthcare, science, or business school with coursework in a field related to the medical or ambulatory clinical experience; one to three years of related and progressively more responsible or expansive work experience in performing tasks related to the area of employment; experience with electronic medical records and electronic scheduling systems; experience in data analysis and reporting; experience in healthcare quality programs; experience in the use of spreadsheet, word processing, and database software; or an equivalent combination of education and experience.
Desired Qualifications
To be considered for the Community Health Worker position at MSU Health Care Inc., candidates should meet the following requirements:
Certification: Must have a valid Community Health Worker certification or be willing to obtain one within a specified timeframe after hire.
Experience: Previous experience working in a community health or social services setting is preferred. Experience in a healthcare-related role is a strong advantage.
Interpersonal Skills: Excellent communication and interpersonal skills are essential, as the role involves frequent interaction with individuals from diverse backgrounds.
Cultural Competency: Understanding and appreciation of cultural diversity and the ability to provide culturally sensitive care.
Organizational Skills: Strong organizational and time management skills to manage a caseload effectively.
Computer Proficiency: Basic computer skills for data entry and reporting.
Driver's License: A valid driver's license and access to reliable transportation is required for community outreach activities.
