THE LIBRARY DEPARTMENT
The Community Health Advocate is a vital member of both our Community Hub team and the South Lake Tahoe Library which overlooks beautiful Lake Tahoe and is frequented by visitors year-round. Community Hubs are collaborative efforts located within our Library branches and staffed by a team specializing in family engagement, early literacy, and community health that are focused on children, families, and individuals across the lifespan. Each Community Hub team is dedicated to building resilient communities by helping individuals including seniors, veterans, families, and caregivers gain access to needed resources, including informal community supports and formal services.
THE OPPORTUNITY
Perform a variety of public health duties related to community outreach and education regarding basic public health practices; perform referral based client follow-up and assists in organizing community networks.
The selected candidate will have the opportunity to:
Disseminate health education information and provide instruction regarding basic public health practices relating to communicable disease, immunization, nutrition, and sanitation.
Collect, evaluate and discuss findings regarding health needs and cultural attitudes of the community with technical, professional and management staff in the Public Health Department.
Collaborate in the development of culturally attuned services and outreach strategies regarding public health practices.
Provide appropriate individual, community, and population based interventions and follow-up to improve identified health issues as outlined in a plan of care developed by the assigned Public Health Nurse or multi-disciplinary case management team, which includes psycho-social, physical and environmental oriented actions.
Facilitate culturally competent follow-up with referral-based clients, especially those determined to be at high risk.
Conduct basic needs assessments for individuals and/or families.
Connect individuals and/or families to resources either through direct delivery of goods and services and/or referral to outside agencies.
Facilitate community advisory groups to identify community needs.
Develop a working knowledge of what resources are available to individuals and families within our community.
Attend collaborative meetings with partner agencies such as El Dorado County Office of Education, First 5 El Dorado, Health and Human Services Departments, Housing El Dorado Tahoe Coalition for the Homeless, etc.
Data entry & data analysis.
Participate in outreach events such as resource distributions, community events, etc.
Assist patrons with typical library functions such as registering for a library card, checking in/out materials, placing holds, locating requested materials, etc.

Human Resources will assess your application to determine if you are minimally qualified using the following recommendations:
Where college degrees and/or college course credits are required, degrees and college units must be obtained from an accredited college or university. Courses from non-accredited institutions will not be evaluated for this requirement.
Equivalent to two years of college level coursework in sociology, psychology, social services or a related field,
AND
One year of paid or unpaid experience in community outreach activities,
OR
Three years of increasingly responsible experience providing technical and administrative support in a health or social services setting.

The ability to speak and read Spanish in addition to English is required for this position.

Spanish bilingual candidates may be eligible to receive differential pay of $1.00 per hour for all hours in pay status. In order to be eligible to receive such differential, an employee must demonstrate language proficiency acceptable to the Department Head as certified in writing to the Director of Human Resources. The Human Resources Department shall use a verbal and/or written testing process, depending upon the level of bilingual skill required of the employee, to validate the employee's skills. The differential is defined by the Memorandum of Understanding between the County of El Dorado and the Bargaining Unit representing this job classification.
