Shift DetailsM-F 8am to 4pmLocationPhiladelphiaComp:$18.00 HourlyPOSITION SUMMARY: Provide direct job development, job coaching and job support services to individuals with disabilities and overall coordination of their Elwyn (re)habilitation plan.DUTIES AND RESPONSIBILITIES:Conduct client vocational profiles and other assessments based on funding source requirementsCounsel clients to best match their employment interests with their skillsDevelop and provide job readiness training and job club activities to best promote client interest in employmentAssume a lead role in the team's development and review of client service plansSystematically network to seek and develop appropriate employment opportunities that match client interests and skillsConduct job analysis consistent with employer and client needsProvide intensive client on-the-job training, in conjunction with a teaching plan and needs of the employerEstablish metrics to systematically track the progress and success of clientsProvide counseling and supports to the client in order to develop interpersonal and soft skills success on the jobPlan for client transportation and provide travel training as neededTransport clients in agency vehicles as neededIdentify and develop home and natural supports for individuals servedDevelop and implement a coach fading plan which incorporates natural supports that are sensitive to the culture of the work environmentAnticipate and provide follow-along services for clients and their employers in order to maintain employmentConduct client assessment, job find, placement, retention, fading levels and staff service volume according to department and funding source expectationsMaintain clear, active and constructive communication with the client while facilitating communication amongst the client, client home supports, client team members, funding source and monitoring entity representatives, supervisor, co-workers and potential employersComplete monthly reporting based on funding source requirementsComplete annual training requirements as well as any additional training mandated by funding sourcesParticipate in team activities using positive approaches which will build and attain common goalsPerform cardiopulmonary resuscitation (CPR), and crisis intervention using agency-trained protocolsPerform other duties as assignedCONTACTS: Clients, Elwyn staff, funding agencies, client service team member, client employers, employer customers, home and natural supports and potential client employersEDUCATION/EXPERIENCE/SKILLS REQUIREMENTS:High School diploma or equivalent required; Associates degree in psychology, human/social services or a related field preferredOne (1) year experience working with people with disabilitiesDemonstrated ability to work effectively as part of a teamMust be able to perform CPR and crisis intervention techniques using agency-trained protocolsMust possess excellent customer interaction, collaboration, presentation, and written and verbal communication skillsDemonstrated basic experience with Microsoft Office applications, including Word, and Outlook, Elwyn Referral Management System (ERMS) and report-writer experience preferredMust have current, valid driver's license in state of residence, three (3) years driving experience, and acceptable driving recordPHYSICAL DEMANDS/ENVIRONMENTAL PROFILE: See Physical Demands Checklist below which represents the general physical demands and environment conditions to which the employee holding this position must meet, in addition to the duties and responsibilities listed above, to successfully perform the essential functions of this job.
