Our clinical social workers have the knowledge andunderstanding needed to provide psychosocial services to children and familiesexperiencing the difficulties associated with illness, hospitalization andoutpatient care. Please identify yourpreferences in the questions within the application.Please note, these positions are subject to on-call shifts and rotations asneeded.Make an Impact (Curious about the impact Clinical Social Workers have? )Qualifications:Clinical Social Worker:Education: Master's degree in Social WorkExperience: Equivalent of 1 years of full time work experience providingpsycho-social assessment and interventions to patients and families.Licensure(s): Licensed Social Worker (LSW) in the state of Colorado (CO)Entry Level:Education: Master's Degree in Social WorkLicensure(s): Must be able to attain the licensed social worker(LSW) in the state of Colorado(CO) within 1 year of employment.Job Highlights:Develops and implements service/treatment interventions that provide avariety of services to meet patient/family needs.Monitors progress, cooperation, and acceptance of services.Develops a therapeutic relationship with the patients and families to assistwith the social, financial, and psychological problems associated withhospitalization and/or outpatient treatment.Conducts initial needs assessment to gather information on thepatient/family's medical, social, and financial history.Serves as a liaison with multi-disciplinary care teams and external communityagencies to identify, implement, and monitor treatment goals.Facilitates staffing, care conferences, etc to coordinate care between thetreatment team.Compiles and prepares case records, reports, and documents that complywith state and federal standards in providing case notes, treatment plans,and evaluations.
