　　The Client Assessment Specialist offers an exciting opportunity to positively impact your career and community!

　　Responsibilities

　　Conducting in-home assessments for LT-PCS applicants and participants.

　　Providing HCBS program choice counseling to requestors.

　　Providing an overview of the Long-Term Care Program to applicants (Rights and Responsibilities).

　　Developing a needs-based and person-centered plan of care.

　　Completing client monitoring surveys as part of quality management.

　　Requirements

　　Have a bachelor’s degree in health/human services, social work, nursing or related.

　　Possess a minimum of two years of health related social and/or human services experience.

　　Show a minimum of one year of experience working with older adults or people with disabilities.

　　Need to reside in Louisiana.

　　Have a valid driver’s license.

For this position, actual salaries will vary and may be above or below the range based on various factors including but not limited to location, experience, and performance. In addition to base pay, this position, based on business need, may be eligible for a bonus or incentive. The estimated compensation range for this role is $15-17 per hour.
