JOB REQUIREMENTS: Overview Cares for patients in the skilled nursingfacility settings under direction of nursing staff, by performing thefollowing duties, supporting the nursing philosophy. May includeweekends, holidays, day, evening, night, and/or on-call hours.Responsibilities Provides resident care as assigned and per individualresident routine, according to standards of care, under CMS Federal andState Regulations for Skilled Nursing Facilities, and facility policiesand procedures. The ability to identify and document in the medicalrecord the resident\'s usual and current cognitive patterns, mood andbehavior, and whether these present a risk to the resident or others,identifying and documenting how the resident typically displays personaldistress such as anxiety or fatigue, recognizing a change in theresident and the resident\'s functioning and the ability to describe andreport such changes to the nurse such as negative verbalizations andvocalizations, facial expressions of grimacing, frowning, or fright,changes in gait, skin color, vital signs, behaviors such as resistingcare, distressed pacing, irritability, depressed mood, and or decreasedparticipation in usual physical and/or social activities, loss offunction or inability to perform Activities of Daily Living, difficultyeating or loss of appetite and difficulty sleeping according to CMS RAIrequirements. Performs and documents daily range of motion exercises,assisting the resident to transfer from bed to wheelchair, assistingresidents daily with bathing, dressing, grooming, transfers andambulating through the facility, toileting, and eating. Assistsresidents to attend activities of their choice Grooming residents asthey wish to be groomed (e.g., removal of facial hair for women,maintaining the resident\'s personal preferences regarding hairlength/style, facial hair for men, and clothing style.) The ability todescribe the care, services and expected outcomes of care they provide;have knowledge of the care and services provided by therapists; have anunderstanding of the expected outcomes of this care and understand therelationship to the care they provide. The ability to identify anddocument , to the extent possible, factors that may underlie theresident\'s expressions of distress, as well as applying knowledge oflifelong patterns, preferences, and interests for daily activities toenhance quality of life and individualize routine care. The ability toidentifying and understanding that the resident with dementia may beresponding predictably given the situation or surrounding.
