　　Provides assistance with activities of daily living in accordance withthe individualized plan of care (turning, repositioning, feeding,bathing)

　　Observes and records changes in condition of residents and reports tothe licensed nurse

　　Completes required documentation in the electronic clinical record

　　Obtains and reports vital signs and weights on residents as assigned
