　　Department: MedSurg Swing

　　Schedule: PRN Primarily 6:45p-6:45a, but could rotate to day shift as well. Four (12-hour) shifts commitment in a six-week schedule period required. Two of the shifts must include weekend days/nights. One summer and one winter holiday commitment.

　　Responsibilities

　　Assist with patients/participants care activities.

　　Assist with procedures ordered by physician and supervised by a Registered Nurse (RN).

　　Accommodate patients/participants with tending to personal care, activities of daily living and transfers/transport.

　　Report findings or changes in physical, mental and emotional conditions to nursing staff.

　　Assist with keeping patients/participants rooms clean and orderly.

　　Monitor patients/participants environment for safety hazards and emergency situations and initiates corrective action as appropriate, as needed.

　　Requirements

　　Licensure / Certification / Registration:

　　BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.

　　Nurse Aide credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date required.

　　Education:

　　High School diploma equivalency OR 1 year of applicable cumulative job specific experience required.

　　Note: Required professional licensure/certification can be used in lieu of education or experience, if applicable.

　　Additional Preferences

　　Previous Medical Surgical or Long Term Care Experience preferred.
