Case Managment Support & Service CoordinatorPsychology - Pennsylvania-Pittsburgh - (24000058) This is a full-time Case Management Support & Service Coordinator position in the Department of Psychology at the University of Pittsburgh. The candidate will be part of a multidisciplinary team that ensures the safety and wellbeing of parents with children throughout Allegheny County. The candidate will display and exhibit high quality service and be key in support and service offering, connecting, and coordinating families to resources, services with ongoing follow up and care management. In addition, the Support & Service Coordinator will assess the overall needs the parent has for specific resources provided and assist parent with follow through to ensure the service offering was completed. In addition to direct service provision with families, the staff member will work closely with the Hello Baby Team, which includes Family Check-Up clinical staff, the Service and Outreach Coordinator, and our Community Services partners at Allegheny County Department of Human Services
Must have ability to work with individuals who have experienced trauma (trauma-informed), work with individuals with low literacy and mental health challenges- Effectively communicate with parents, staff, community partners/service agencies via phone, in person, in writing, and via text. Navigate various databases, (locating/updating family information, entering data). Communicate knowledge of the numerous and diverse services in and around Allegheny County and be familiar with the many service programs available for families with young children. Willingness to meet parents in their local communities, including home visiting and community agencies - Ability to work a flexible schedule to accommodate parent availability
Job SummaryProvides individual and group counseling, crisis intervention, and drug and alcohol assessments; creates assessments and therapeutic courses of action. Creates and facilitates student psycho-educational, prevention, and support groups as well as student success programming. Instructs undergraduate classes; administers campus-wide initiatives. Assists to train interns and staff.
Essential Functions Connecting families with intervention programs. Describing the plethora of resources that are available to help families with young children. Meet with parent/guardian of the families in their home and/or public community setting. Facilitate community connections -- liaising with community providers. Work closely with Family Check-Up interventionists to collaborate with various services being provided to service/assist families. - Support families with completion of paperwork for services, such as housing, food, etc., and obtain much needed documentation that accompanies each application. Attend training and staff meetings both in-person and remote. Actively participate in weekly reflective supervision and other meetings as needed Accurately log all correspondence and services provision with families. Following up with families and intervention providers to ensure connections have been made and follow through with planning and family goal attainment. Providing correspondence/follow up to FCU Coaches as needed re: their families and to Supervisor or Hello Baby personnel as needed/requested.
Physical Effort Position is mostly sedentary and will require an individual to lift/carry goods (up to 30 pounds) needed for families. Individual should be able to move about between offices and walk stairs as necessary traveling to families' homes and agencies throughout Allegheny County.
