Description
The Substance Use Disorder Case Manager will coordinate the plan of care for program participants with substance use issues. The SUD Case Manager will collaborate with community partners to assist in providing referral and easy access to outpatient SUD services in the community. The SUD Case Manager shall maintain a close working relationship with all members of the participants’ team to ensure delivery to participants comprehensive treatment in an efficient and expedient manner with appropriate follow-up.
Examples of Duties
Engage priority populations identified through access departments, referrals from various community social services agencies, probation officers, Michigan Department of Corrections, and calls initiated by the consumer.
Conduct screenings
Work closely with SUD Treatment and Prevention Providers to ensure effective and efficient processes are in place for timely admissions to referred services.
Directly assist consumers in getting timely intake appointments to providers and link individuals to local resources to support various priority populations needs.
Maintain data tracking for priority populations individuals.
Assist in identifying training needs to increase the knowledge of all staff.
Monthly reporting of progress clients who was referred to services.
Provide mental health and SUD updates.
Activities/assignments may change at any time with or without notice.
Conduct outreach activities.

Minimum Qualifications
Bachelor's or Master's degree Public Health, Business Administration, Health Science Administration or other related field, with two years experience in community engagement, community organizing, and project coordination and implementation.
Preferred:
Knowledge of applicable laws, regulations, policies and procedures and ability to interpret and disseminate information.
Ability to organize, prioritize, and plan work activities and projects to meet deadlines.
Ability to make timely decisions using sound and accurate judgment with specified constraints.
Ability to work independently and maintain working relationships to collaborate as part of a team
Knowledge of mental health code
Knowledge of recipient rights code.
Ability to behave ethically, honestly, and with integrity.
Equivalent combinations of education and experience may be substituted to meet the education and experience requirement of this position

City Employees Retirement System
As a regular City employee you automatically become a member of the General Retirement System. This entitles you to a retirement allowance after:
