The Care Management Support Assistant contributes to administration of care management. Provides non-clinical support to the assessment and evaluation of members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Care Management Support Assistant performs varied activities and moderately complex administrative/operational/customer support assignments. Performs computations. Typically works on semi-routine assignments.
The Care Management Support Assistant Decisions are typically focus on interpretation of area/department policy and methods for completing assignments. Works within defined parameters to identify work expectations and quality standards, but has some latitude over prioritization/timing, and works under minimal direction. Follows standard policies/practices that allow for some opportunity for interpretation/deviation and/or independent discretion.

Required Qualifications
At least 2 years or more of Administrative Support experience preferably in the Healthcare Industry
Must be fully Bilingual English and Spanish
Knowledge of MS Office (Word, Excel, Outlook, Outlook)
Familiarity with care and well-being resources (Assisting our patients with social service resources like housing, food stamps, etc).
Must be passionate about contributing to an organization focused on continuously improving consumer experiences
Preferred Qualifications
Knowledge of Medicare/Medicaid guidelines
Knowledge of Medical Terminology
Associate's Degree
Microsoft Access proficiency

Working hours : Monday to Friday 8AM-5PM
Location: Must be flexible to travel to 4 Jacksonville Centers: (Callahan, University, Gateway and Normandy)

This role is considered patient facing and is part of Humana/Senior Bridge's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
According to Humana's COVID-19 policy, vaccination and masking is required regardless of vaccination status (with exception for eating and drinking) for associates working in a clinical setting where patients receive care (e.g., pharmacy retail locations, clinics, home care). The policy only requires the primary series alone (first and second vaccine). Boosters are encouraged, but not required. Interview Format: MODERN HIRE

If participating in a pre-recorded interview, you will respond to a set of interview questions via your phone. You should anticipate this interview to take approximately 10-15 minutes.
If participating in a SMS Text interview, you will be asked a series of questions to which you will be using your cell phone or computer to answer the questions provided. Expect this type of interview to last anywhere from 5-10 minutes. Your recorded interview(s) via text and/or pre-recorded voice will be reviewed, and you will subsequently be informed if you will be moving forward to next round of interviews.
