About Care Management Lead

  Job Summary

  Oversee and supervises the daily activities of the Care Management Department.

  Job Duties

  Collaborates with management to design and develop Care Management in support of the goals of the department, division, hospital and system.

  Maintains operational responsibility for daily staffing plans to support the department including daily assignments to support delivery of quality safe care.

  Allocates staff coverage to provide optimal utilization of staff resources.

  Supports the department's operating in a cost effective manner.

  Establish collaborative working relationships within and outside the organization.

  Supports the development and maintains beneficial internal and external relationships in service of the department's mission.

  Provides input into the performance appraisal process in an accurate and timely manner.

  Manages a defined caseload according to defined case manager responsibilities.

  Serves as a mentor and coach for staff.

  Evaluates staff development needs and provides opportunities for ongoing professional development.

  Work is typically performed in a clinical environment. Accountable for satisfying all job specific obligations and complying with all organization policies and procedures.   Position Details

  Education

  Master's Degree-Social Work (Required)

  Experience

  Minimum of 5 years-Clinical (Required), Minimum of 1 year-Managerial/Supervisory (Preferred)

  Certification(s) and License(s)

  Licensed Master Social Worker - Default Issuing Body
