Description Lutheran Social Services of WI andUpper MI is currently seeking Care Coordinators to join our newMilwaukee County team! This employee will be required to be fluent inSpanish and English. The Children\'s Long Term Support (CLTS) Waiver isa federally funded Medicaid program serving the needs of children whohave a developmental, physical, or a severe emotional disability. CLTSCare Coordinators serves as the main point of contact in helpingfamilies obtain CLTS services necessary to help keep their childrensafely in a home/community setting. The position is full time, Mondaythrough Friday, first shift, and requires flexibility to meet the needsof the families. Full Time Benefits are available!
