Intermountain Health is seeking an Advanced Practice Provider, House Calls to join our team in Nevada. Conduct patient visits in the home including medication reconciliation. Manage existing conditions; diagnose and formulate plan of care. Write referrals, renew medications, order diagnostics, and manage results. Complete POLST (Physician Order for Life-Sustaining Treatment) for patients as necessary. Advanced directive and advanced life care planning activities are a priority in this position.
This position is bringing care to the most vulnerable patients in their homes to prevent them from going to the hospital. This is a highly successful team that is improving lives daily.
Practice Information
Work in collaboration with a supportive care-team
Work Schedule: 4 days a week / 7AM-6PM
This position requires you to travel within Nye and Clark Counties
Must be able to manage complex cases, which includes difficult patients.
Avg Daily Patient Census: 7/day Senior population and some younger patients who are medically disabled and on Medicare supplemental program.
No Call
Experience with treating patients with multiple co-morbidities
Possess advance critical thinking and strong communication skills
Requirements
Current Nevada R.N. and N.P. license or Physician Assistant or Adult/Gerontology, NP
Certificate of completion from an accredited Nurse Practitioner Program or Physician Assistant Program
Current Nevada DEA certificate required prior to start date
BLS certification
At least 2 years’ experience practicing as a Nurse Practitioner in Primary Care, House Calls or Hospice
Must have valid driver’s license and insurance
Must be comfortable conducting home visits with high-risk patients
Two years of experience in Primary Care or Hospice
Preferred
Spanish-Speaking
Advance practitioner with cardiology background

Physical Requirements:
Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.
and -Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.
and -Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.
and -Expected to lift and utilize full range of movement to transfer patients. Will also bend to retrieve, lift, and carry supplies and equipment. Typically includes items of varying weights, up to and including heavy items.
and -Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling gurneys and portable equipment, including heavy items. Often required to navigate crowded and busy rooms (full of equipment, power cords on the floor, etc).
and -May be expected to stand in a stationary position for an extended period of time.
