Position Overview:
Utilizes extended and expanded skills, experience and knowledge in assessment, planning, implementation, diagnosis and evaluation of the care required within an acute care specialty. Accountable for the coordination of nursing care, including direct patient care, patient/family education and transitions of care. In partnership with the care team, the RN delivers holistic and individualized care to patients in an assigned specialty area. Develops, implements, and coordinates a plan of care that incorporates psychological, sociocultural, spiritual, economic, and life-style factors. Fosters and maintains collaborative relationships between patients, their family/support group, physicians, and other healthcare providers through timely and effective communications.
Job Description :
Delivers nursing care for patients with complex needs within an area of specialty. Provides direct care to patients with a variety of needs utilizing the nursing process and within the scope, ethics, and standards of practice. Utilizes the nursing process for assessment, planning, implementation, and evaluation to provide safe, therapeutic care. Identifies goals, formulates plan of patient care and evaluates outcome. Consults with multidisciplinary health care team in regard to nursing assessment findings. Conducts clinical rounds, provides emergency care and administers medications to patient. Operates equipment required for patient care according to manufacturer's recommendations.
Performs and assists with procedures and treatments on unit including but not limited to changing dressings, feeding patients and providing pre/post operative care. Creates written, individualized plan of care based on priority of patient problems, anticipated length of stay, input from patient and family and other members of the health care team. Systematically and continuously collects, assesses and documents data in collaboration with the multidisciplinary team to provide therapeutic care for the patient and family. Foresees potential problems and adapts priorities, plans independently and without outside direction.
Provides counseling and teaching for clinic patients and their family/significant other. Develops and implements a teaching plan of care, informing and educating patients and their family to ensure participation in their care and decision making. Instructs patients and/or family members/significant others regarding treatment, medication, diagnostic procedures and follow-up visits. Observes and identifies underlying and contributing factors/concerns, especially socioeconomic, cultural, physical and language barriers.
Anticipates, plans, directs and evaluates clinic activities. Analyzes and assists in resolving problems resulting in improvement of patient care, practice and system operations. Coordinates planned nursing care with other health care team members. Contributes to and supports the effective management of the clinic. Makes sound judgments in decisions and problems that are complex and require an interdepartmental, intradepartmental, and interdisciplinary approach. Oversees delegated care based on abilities of clinic personnel and patients' medical condition. Sets priorities and makes adjustments as needed.
Participates in activities that support the advancement of nursing practice through best practice literature, professional organizations, research and hospital nursing committees and councils. Evaluates and updates materials for clinic patients. Acts as preceptor for other clinic staff, students and fellows.

TYPICAL EXPERIENCE:

Department: Wound Care, 2 years' experience as a Wound Care RN in a clinical, active in the care of wound care patients.
Department: OB-GYN, 2 years' experience as a Midwife in an acute care or clinic setting.
Department: Cardiac Rehab, 1 year experience in a medical/telemetry unit
SKILLS AND KNOWLEDGE:
Demonstrated advance knowledge and competence with regard to area of specialty, nursing theories, concepts and practices; medical terminology; anatomy; and physiology.
Advanced knowledge of the healthcare industry, safety precaution policies, best practices regarding patient care and privacy, and changes in local/state/federal regulations.
Possess written and verbal communication skills to explain sensitive information clearly and professionally to diverse audiences, including non-medical people.
Time management and organizational skills, including the ability to prioritize patient care, assignments and work within standardized policies, procedures, and practices to achieve objectives and meet time-sensitive activities.
Basic knowledge of computer applications, such as Microsoft Office Suite (Word and Outlook), Electronic Health Record.
Work independently as well as be part of the team, while accomplishing multiple interventions in an ever-changing environment.
Ability to communicate and engage team members, patients and families in a variety of stressful situations.
Use existing procedures and critical thinking to identify and solve routine or complex problems.
Ensure the privacy of each patient’s protected health information (PHI).
Build collaborative relationships with peers and other healthcare providers to achieve departmental and corporate objectives.
Prioritize assignments and work within standardized policies, procedures, and scientific methods to achieve objectives and meet deadlines.

Pay Range is $79.37 to $104.76 / hour
The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, department equity, training and organizational needs.
