Status: Part time / Per Diem

　　Wages: $17.50 per hour

　　Description:

　　The successful candidate will be expected to:

　　Ensure participants’ care plans are being implemented on an ongoing basis.

　　Provide monitoring and oversight to all participants.

　　Ensure a meaningful, enriching environment and experience to participants in a Social Adult Day Program.

　　Assist participants with escorting on and off transport vehicles.

　　Perform daily pre trip and post trip vehicle inspections.

　　Read and interpret maps and driving directions to plan the most efficient route for participants.

　　Fuel the vehicle.

　　Ensure periodic scheduled vehicle maintenance is completed and reported.

　　Provide safety briefings to participants and staff.

　　Keep the assigned vehicles clean inside and outside.

　　Serve as back up to the Full Time Driver working in the Many Hearts program.

　　Educational Requirements:

　　A High School Diploma or equivalent is required.Knowledge, Skills & Abilities:

　　Driving experience in a work setting preferred.

　　1-2 years of experience working with individuals with disabilities.

　　Knowledge of Independent Living Philosophy, HIPAA and operations and features of assigned vehicles.

　　Ability to lift up to 75 pounds.

　　A valid, clean, NY State Driver’s License is required.

　　Ability to establish effective relationships, responsiveness to working with the elderly population.

　　Ability to prepare concise and accurate records.
