JOB REQUIREMENTS: Approximately 75%: Screen all new requests forinformation and services, and document all reports accordingly. Conductinterviews of a sensitive nature, on the phone and in person, withindividuals who present themselves for social/human services deliveryprogramming. Assess and analyze situations to determine the social,economic, and behavioral problems and needs of the clients. Referclients to the service providers based on the identified needs of theclient; And, coordinate the referrals to other agency service providersfor new clients. Learn, interpret, and apply laws, regulations, andagency policy as it applies to the assigned program areas. Perform otherduties to support the Family Services Unit, for example, casemanagement, assessments, foster parent recruitment, supervise familyvisitation. Perform duties and document in eWiSACWIS the matters ofinvestigations, client case management, social work plans, clientprogress notes, and other records; Provide outreach, aftercare, followup and community liaison where appropriate. Develop and maintain acurrent resource guide of other service providers locally, regionally,and state and national resources. Approximately 25%: Assist theCoordinated Service Team Program (CST), and other program initiatives.Identify and bring together a team of people that will collaborativelywork with the child and family, and provide process orientation to thefamily and to service providers who are new to the process. Togetherwith team partners, conduct a comprehensive and multi-dimensionalsummary of strengths and needs of the child and family. Coordinate theimplementation of the Plan of Care and monitor ongoing delivery ofservices. General Responsibilities: Attend staff meetings and individualsupervisory conferences. Participate in the On-call and after hoursservices rotation as defined by management, so as to provide crisis andemergency intervention services on behalf of agency population groups asmember of the agency\'s on-call network. Participate in the QualityAssurance Review process. Attend in-services training, workshops, andconferences as it pertains to assigned duties. ***** OTHEREXPERIENCE AND QUALIFICATIONS: A minimum of one year of work-relatedexperience preferred, preferably involved in association with acommunity-based social/human services program. Position requiresconsiderable ability to communicate effectively, both orally and inwriting and ability to work cooperatively with other agencies in theprovision of services. The candidate must have the ability to establishgood rapport quickly and the ability to work effectively with a broadrange of individuals. Must have excellent computer stills, including butnot limited to use of database systems, G Suite, Microsoft Windows,Microsoft Office (Word, Outlook, Excel, etc.). Preferred priorexperience in child protection services or mental health treatment in anoutpatient setting. Physical Requirements: Work is normally performedfrom a sedentary position. Ability to function primarily in situationsencountered in a normal office setting. However, due to the crisisresponse component of this position, it can be very stressful at times.Activities include frequent verbal and written communication utilizingphone systems and computer applications as well as face to face withindividuals.
