Our compassionate services also reach seniors living in the broader Twin Cities area through home care, outpatient therapies and wellness programs. Rooted in history, Saint Therese of New Hope is our largest community. This care center has 168 skilled nursing beds for long-term and transitional care. In addition, the Residence at Saint Therese of New Hope houses 225 independent apartments and suites for assisted living and memory care. Responsibilities: Evaluates social factors relating to rehabilitation requirements, and provides personal counseling and referral services to residents of the facility. Evaluates and attends to the psycho-social needs of residents.
