Minimum Qualifications • Reachable on the Civil Service List for Social Work Assistant 1 or 2; OR• Currently serving as a permanent Social Work Assistant 1 or 2 eligible for transfer; OR• One year of permanent competitive service in a title eligible for transfer under Section 70.1 of the NYS Civil Service Law.MINIMUM QUALIFICATIONS FOR PROVISIONAL APPOINTMENT:If the Eligible Lists produce two or fewer qualified and interested candidates in a location, the agency may nominate, at its discretion, a candidate for provisional appointment to the position. To be qualified for provisional appointment, candidates must meet one of the following conditions:Either A. a bachelor’s degree or higher in social work; Or B. a bachelor’s degree or higher in a human service area* and one year of post-bachelor’s social work experience.*Degrees in the following human service areas are considered qualifying: The professional disciplines of art therapy, audiology, dance therapy, drama therapy, music therapy, nursing, nutrition, occupational therapy, physical therapy, psychology, social work, speech/language pathology, and therapeutic recreation. Also included are related academic disciplines associated with the study of: human behavior (e.g., gerontology, human services, sociology, speech communication); human skills development (e.g., counseling, education, human development); or any other study of services related to basic human care needs (e.g., community mental health, rehabilitation counseling).Examples of non-qualifying degrees include but are not limited to: Anthropology, archeology, communication, criminal justice, fine arts, philosophy, and political science. Qualifying social work experience involves activities such as assessing individual and family needs, strengths and limitations; assisting in the development of treatment plans; providing counseling to promote mental, emotional, physical and social functioning; helping individuals and families access supportive services; coordinating and evaluating services, programs and support systems; and providing support, advocacy and referral as appropriate. Examples of non-qualifying experience include providing direct hands-on care to patients/consumers by assisting with bathing, dressing, mobility, eating, and other activities of daily living; transporting/accompanying patients/consumers to appointments or activities; planning, organizing and participating in recreational activities for patients/consumers, such as sports, games, creative arts, and/or clubs; and performing safety and/or security related functions for a facility.
Duties Description PRIMARY DUTIES AND RESPONSIBILITIES:• Facilitate OPWDD's Mission by maintaining individuals in Family Care and helping them to live richer, more independent lives, enjoy meaningful relationships with friends, family and others, and fully participate in their communities.• Complete monthly visits to Family Care homes to review records and ensure that all program and environmental are being met • Ensure the Family Care Provider receives and understands the current Staff Action Plan, and documentation of services is completed and accurate• Process TABS payments and other reimbursements made to Family Care Providers• Review Personal Allowance ledgers and receipts monthly, count cash on hand quarterly, complete annual Personal Expenditure Plan (PEP)• Adhere to all regulatory and agency policies and procedures related to Family Care• Communicate with the Family Care Coordinator and administrative staff on any issues within the Family Care Program that might compromise the individual’s safety in the home• Participate in team meetings to develop the Staff Action Plan, Safeguard Summary, and Life Plan• Communicate with the team including the Care Manager, regarding recommendations to address the individuals’ needs and safeguards • Field work is required• Other duties as assigned by Supervisorhttps://www.linkedin.com/company/nys-office-for-people-with-developmental-disabilities/posts/?feedView=all#LI-VC1
