Adecco Medical & Science is looking for a Senior Wellness Coordinator for our client in Minneapolis, MN ! This is a remote/work from home position.

　　Type: 5 month contract with the opportunity for extension or full time offer

　　Schedule: Range between 8:00AM - 6:00PM MST

　　Pay: $24.31 - $26.31 based on experience

　　Responsibilities for Senior Wellness Coordinator

　　· Makes outbound calls from an automated dialing system to members who are diagnosed with behavioral health needs

　　· Contacts the aftercare providers (individual practitioners and/or Community Mental Health Centers) and records aftercare appointments

　　· Determines if the enrollee is aware of his / her aftercare plan and provides assistance with any additional behavioral health needs

　　· Assists enrollees with understanding and locating community support services

　　· Answer provider and member calls from a queue

　　· Process service request forms for PAs/Salesforce

　　Requirements for Senior Wellness Coordinator

　　· High school diploma or GED

　　· Inbound and outbound call experience

　　· Microsoft Word and Excel experience

　　· Microsoft Teams experience

　　· Excellent verbal and written communication skills
