Occupational Therapist The Occupational Therapist (OT) is an employee who provides occupational therapy services to Amada Home Health, Inc. clients as ordered by the physician. REPORTS TO: Clinical Supervisor SUPERVISES: Occupational Therapy Assistants for direct client service as assigned by supervision QUALIFICATIONS: * Must be a graduate from an occupational therapy educational program accredited by the Accreditation Council for Occupational Therapy Education (ACOTE) and eligible to take or has successfully taken the entry-level certification examination for Occupational Therapy by the National Board for Certification in Occupational Therapy, Inc. (NBOCOT) * Possesses and maintains a current state certification/license as an OT * Maintains current liability and malpractice insurance * Maintains CPR certification * Possesses effective communication skills and ability to work with individuals from a variety of backgrounds and educational experiences * Possesses good time management and work ethics * Possesses a current driver’s license and a dependable insured automobile * Possesses self-direction and ability to make independent client service judgments * Presents a professional appearance and promotes a positive work environment * Demonstrates computer literacy * Provides written proof of health status and ability to lift, bend, and squat the level and amount determined by each job assigned RESPONSIBILITIES: * Demonstrates support for the agency mission statement to promote quality, efficient, comprehensive, and effective client service. * Participates in and supports the agency’s performance improvement activities. * Participates in orientation, competency testing, and in-services as required. * Performs comprehensive initial and on-going occupational therapy assessments per standards of practice on each assigned home health client and develops the plan of service in collaboration with client, RN Case Manager, and physician. * Regularly evaluates the effectiveness of each assigned client’s plan of service, response to treatment, revision needs, discharge plans, and progress to client goals. * Establishes goals to enhance the rehabilitative ability of the client, which may include but not be limited to: neuromuscular facilitation/inhibition; perceptual, prosthetic, and kinetic treatment; orthotics/splints; activity of daily living training; and related to needs. * Uses a framework of assessment, implementation, coordination, and evaluation providing outcome-based services for an assigned caseload from admission to discharge. * Complies with all home health regulations and standards and agency policies and procedures. * Appropriately seeks supervisory assistance as necessary to effectively perform duties. * Assesses and reports client pain score to RN Case Manager. * Maintains confidentiality of client, personnel, and agency operations. * Using agency documentation materials and/or computer-based software to record client treatment activity in a timely manner including client response to interventions. * Promotes client, personnel, and office safety and utilizes universal precautions. * Meets productivity requirements established for home health occupational therapist. * Supervises assigned Occupational Therapist Assistants (OTAs) every 30 days by establishing and/or updating a plan of service in conjunction with the client/representative and evaluating compliance and progress to measurable goals. * During client/representative interactions identifies cultural, spiritual, verbal, and non-verbal patterns indicating a need for counseling and suggests referral to RN Case Manager. * Assesses caregiver’s ability to manage client occupational therapy needs. * Evaluates the safety of the home environment and recommends to client/representative potential solutions for identified concerns. * Uses equipment properly and seeks training if necessary. * Provides education, advice, and consultation with client/representative and agency personnel as needed. * Participates in PIP quarterly record review as assigned. * Completes yearly clinical evaluation for assigned OTA. * Serves on PAC, if applicable. * Presents a professional appearance and promotes a positive work environment. * Performs other duties as requested.
