A Brief OverviewThe Home Care Clinical Nurse is responsible for providing nursing care and services to home care patients in accordance with the CHOP, CHHC standards of practice, policy and procedure and within all regulatory and accrediting agency guidelines. Utilizes the nursing process to deliver nursing care to patients and families.
What you will doPractice: Provides competent care for patients and families based on experience.Nursing practice and documentation is in accordance with CHOP, CHHC standards of care, policy &procedure as well as all regulatory and accrediting agency guidelines.Participates in on -call rotation to meet patient care needs.Professionalism/Leadership: Demonstrates competence in professional practice.Research: Demonstrates an awareness of research and its value to clinical practice.Participates in efforts to apply research findings to practice.Learning Orientation: Participates in the development of professional and educational activities.Examines successes and failures to improve performance.Maintains a blame free environment.Able to identify own learning needs. Seeks out opportunity to meet needs and attends all required programs.Resource Management: Fully understand appropriate utilization of financial resources.Participates in promoting quality, efficient, and cost effective care.Performs other duties as assigned by Home Care Management.Education QualificationsTechnical Diploma Nursing RequiredBachelor's Degree Nursing PreferredExperience QualificationsPediatric acute, subacute, other inpatient or Home Care experience RequiredAt least two (2) years of inpatient nursing with pediatrics/adults PreferredAt least one (1) year of home care experience PreferredSkills and AbilitiesValid Driver's licenseStrong interpersonal and customer service skillsStrong verbal and communication skillsStrong clinical judgment skillsAbility to work independentlyAbility to work as a team player in a fast paced environmentData entry skillsLicenses and CertificationsRegistered Nurse (Pennsylvania) - Pennsylvania State Licensing Board - upon hire - Required andRegistered Nurse (New Jersey) - New Jersey State Licensing Board - within 9 months - Required
