   Position Summary

  The Executive Director of Population Health is the key business leader who oversees the organization's medical services programs and is responsible for oversight, support, and maintaining these programs, as well as enhancing population health initiatives in service of value-based care principles and outcomes.

  Together with the clinical teams, product and analytics teams is responsible for developing and implementing integrated models of care management programs and services that control medical costs, create clinical efficiencies, and improve beneficiary health. In addition, the Executive Director of Population Health applies their understanding of clinical practice, healthcare policy and financing, risk-based provider relationships, and evolving reimbursement models and integration strategy to promote partnerships with providers, third-party specialized vendors, community organizations and other key ACO stakeholders.

  Strong leadership and team building skills within a highly matrixed organization are required. Prioritization and communication skills are critical to success, as managing the expectations of other leaders across the organization in support of their strategies is imperative.

  The Executive Director of Population Health is jointly accountable for achieving business results in areas of responsibility within the ACO and in support of CVS ACO's strategic goals. Areas of functional responsibility include clinical quality, care management and care coordination support, clinical integration with ACO provider arrangements, and partner programs. Additionally, driving outcomes using targeted analysis of population health and collaborative provider partner relationships is required. Experience implementing advanced analytic workflow changes and innovative population health solutions will be preferred in this search.

   You'll make an impact by:

  • Working collaboratively with leaders across CVS ACO to achieve strategic goals as related to the development and implementation of Population Health Management strategy and programs.

  • Providing thought leadership and strategic subject matter expertise on matters related to Population Health Management

  • Working collaboratively with enterprise teams within the CMO organization to develop consistent care management productivity and efficacy, and reporting and analytics to measure effectiveness of existing programs and development of potential new program interventions

  • Providing leadership and support to teams of clinical leaders and professionals, organizing talent and resources to deliver results and achieve strategic goals while establishing/maintaining a collaborative and mission-driven team culture

  • Serving as p rimary support for integration of care management strategy and clinical operations with ACO partners and other provider organizations, working in a dyad model with clinical managers, to meet long term goals of collaborative, value-based reimbursement arrangements

  • Actively collaborating and partnering with Provider Transformation Leads, the Chief Medical Officer, and Senior Leadership to ensure alignment of clinical initiatives

  • Participating in the delivery of initial and ongoing staff education programs. Actively promotes and supports professional advancement and career development among team members

  • Representing CVS ACO to external constituents

   Required Qualifications:

  The successful candidate will have a strong work ethic, be a self-starter, and able to be highly productive in a dynamic, collaborative environment. The ideal candidate will have the following experience/knowledge/ability:Minimum of 10 years of related experience in progressive leadership roles in managed health care, clinical programs, services, or administrationMinimum of 3 years successful performance in a clinical leadership or management roleExperience working with provider entities and ACOs under value-based arrangements and progressive payment modelsKnowledge of clinical management, project management, and process improvement and design requiredSuccess in establishing external relationships with community stakeholdersDemonstrates a commitment to diversity, equity, and inclusion through continuous development, modeling inclusive behaviors, and proactively managing bias Preferred Qualifications: Recent or current clinical care delivery experience Experience with providing services in Medicare and/or programs for dually eligible beneficiaries Education: Advanced Degree in a clinical discipline Pay Range

  The typical pay range for this role is:

  $178,750.00 - $385,000.00

  This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above. This position also includes an award target in the company's equity award program.
