Details about this exciting opportunity:
Full-Time Care Manager to work in our Certified Behavioral Health Clinic (CCBHC) in Yonkers, NY.
This role will be part of a small multidisciplinary team that provides support and assistance to consumers (aged 4 throughout the lifespan).
Compensation: $48,000 - $50,000/year.

Responsibilities include (but are not limited to):
Enrolling individuals into wrap around services, providing initial engagement and assessment for COPS Referrals and New Clients entering services.
Completion of initial and ongoing NOMS assessments.
Conducting Social Determinants of Health assessments.
Collaborating with clients & clinicians to create care plans, tailored to the individual.
Coordinating with care team & following up on referrals.
Primary care management.
Participating in multidisciplinary care planning meetings & grand rounds.
Assisting the team with crisis intervention and stabilization, as needed, to support the client.
Participating in outreach and recruitment activities with the team, forging relationships with community providers to help individuals access care.
If you have the following:
Qualifications:
Associate’s Degree in a related field, with 2 years of relevant experience, or a Bachelor’s Degree in a related field.
Driver’s license required.
English/Spanish proficiency is preferred.

Strong knowledge and management of Serious Mental Illness (SMI) and Mental Health (MH) conditions.
Knowledge of community resources.
