  Must provide program supervisor with a mobile number in which you can bereached and be accessible via mobile phone during work hours.

  Provide compassionate care and recovery-oriented services to memberswith mental illness and/or other disabilities.

  Assist members to achieve independent living goals by providing trainingin key areas such as: activities of daily living, budgeting,transportation, developing healthy relationships and other items definedby treatment plan goals

  Provide clients with training, technical assistance and support tomaintain successfully in the community.

  Assist in the development and adherence to a healthy menu (based onclients dietary needs) and in the preparation of meals as indicated bythe clients skill level according to their service and/or treatmentplan.

  Provide and supervise recreational activities with members both on siteand in the community.

  Supervise members in the self-administration of medications. Providetraining/education to them on the process, usage and side effects oftheir medications. Follow-up with supervisor and/or medicalprofessionals around any issues or concerns that happen related tomedications.

  Document services to members as outlined in their treatment plan inindividual or group note forms as applicable. Notes must be compliantwith agency standards and completed before the end of each shift. Assuresafe, clean and secure living environment for all members by assistingthem to complete household chores. Complete cleaning and other tasks asnecessary by the end of the shift.

  Attend all supervisions and trainings as required and/or instructed bythe Program Supervisor or Director of Mental Health.

  OVERALL, JOB PERFORMANCE:

  Adhere to all the Policies and Procedures of Envision Unlimited.

  At all times, maintain confidentiality according to the Illinois MentalHealth code and Confidentiality Act.
