Individual Skill Development andEnhancement includes: training in communication, interpersonal skills,problem solving, conflict resolution, decision-making, self-regulationand other specific needs identified in the youth\'s service plan.Essential Functions: Process assigned referrals, opening electronic andpaper files. Attend initial staffing and follow up case reviews.Maintain face-to-face contact with the juveniles in their homes, atschool, and in the community, as determined at the initial staffing andfollow up reviews. Provide youth with intensive support, mentoring, andmonitoring to assist them with the specific \"areas of focus\" asdetermined in the youth\'s case plan. Provide 24-hour coverage, via cellphones, for after hours skill development and education. Maintainregular, weekly minimum contact with the DHS assigned CCS Coordinator tocollaborate on the youths\' behavior/progress and to report any concernsAttend any/all necessary Department staffings or court hearings, asrequested by the CCS Coordinator. Maintain regular contact with theparents/guardian, as determined at the initial staffing, to collaborateon the youth\'s behavior and to provide ongoing support to thecaretakers. Maintain contact with the youths\' schools, as determinednecessary and appropriate at the initial staffing, to monitor theyouths\' compliance with attendance and behavior at school. Complete allnecessary paperwork and youth reports per policy/procedure, includingcasenotes and billing logs. Enter and maintain database as required bythe program. Document after hour calls on the 24 hour cell phone in alog, update and maintain timely entries, providing the information tothe supervisor upon request. Other Duties and Responsibilities: Attendand contribute to JIS (Juvenile Intensive Services) Department staffmeetings. Assist with answering the office phone when necessary. Otherjob related duties as may be necessary to carry out the responsibilitiesof the position.
