*Summary:\*Provide housing navigation & case management services to participantsco-enrolled in SSVF (Supportive Services for Veteran Families) & VAMedical Center, HCHV (Health Care for Homeless Veterans) OutreachCenter. Work with referred veterans & their families to assess housingresources, needs, barriers & make an appropriate housing plancooperatively with the Veteran & execute plans toward housing stability& permanence. Provide ongoing case management to participants withpending HUD (Housing & Urban Development) -VASH (Veterans AffairsSupportive Housing) housing vouchers. Position requires the ability todrive for company business which includes a valid driver\'s license,clean driving record, & proof of personal auto insurance. Must passdriver record & background check.
*Education &/or Experience:\*Bachelor\'s degree from four-year college/university; or one to twoyears related experience &/or training; or equivalent combination ofeducation/experience.
Essential Duties & Responsibilities include the following:
Work closely with the VA medical center\'s homeless programs &residential & community care programs to ensure that coordination isfacilitated & veterans\' direct care, referral, & follow-up arecontiguous & based on a continuum of services.
Ensure program criteria, referrals, & procedures are maintained forHUD-VASH, SSVF, & program Veteran participants.
Develop working relationships & agreements with other organizations &directly coordinate with the designated PHA regarding housing voucherapplication & award process for each Veteran. Responsible for reviewingthe agreements between Property Managers, PHAs, & the VA to establishappropriate referral sites for placement.
Establish & document participant eligibility. Record information aboutthe participant to identify & coordinate those services that will bemost effective & acceptable to the participant improving housingstability.
Responsible for data collection, administrative record keeping, &performance measurements of participants & that everything is current,accurate, & in compliance.
Regularly conduct & participate in outreach activities, including fieldinterviews, assessments, & referrals for homeless Veterans.
Maintain current, & establish new, relationships with othercommunity-based service providers.
In cooperation, provide services in accordance with SSVF & VA MedicalCenter, HCHV Outreach Center, Goodwill policies, CARF standards, &principles of safety to implement the organization\'s missions & goals.
Facilitate Person-Centered Planning to determine vocational goals,recommend appropriate services to achieve goals, & oversee the program &objectives agreed upon.
Locate landlords willing to lease to clients with housing barriers.
Understand State & Federal Landlord-Tenant laws.
Determine rental & deposit assistance that might be beneficial toprogram participants.
Advocate for the Veteran with community resources, provide communityreferrals & mediation.
