About Family Medicine or Med/Peds Physician - Amplify GR

  Corewell Health West is seeking to hire a board certified/board eligible Family Medicine or Internal Medicine/Pediatrics physician to start up a new Value-Based Primary Care Clinic opening late Fall 2024 to serve the needs of the Boston Square Market residents , and surrounding communities with the collaboration of the local non-profit Amplify GR (https://amplifygr.org/) . in Grand Rapids, MI. The new clinic space will have 6 exam rooms, one procedure room, several flex spaces, and a large classroom which will be used for patient education, cooking classes and community education. This clinic will be decorated with the assistance of local community artists.

  This location will provide Value-Based Primary Care, lifestyle medicine, Healthier Communities services, financial counseling, health insurance literacy education, workforce development, language services, and virtual care using TytoCare technology.

  The ideal physician candidate is one who has experience and passion for treating patients with diverse racial and ethnic backgrounds that have limited resources. In collaboration with Amplify GR, this practice plans to eliminate health disparities by promoting a healthy lifestyle with this underserved community.

  Essential Functions

  Assesses and monitors the medical needs of patients/significant others.

  Provides quality medical services to patients in a cost effective, appropriate manner and ensures coverage of case load at all times.

  Provides input into the development of policies and procedures as they related to medical practice and quality patient care.

  Provides oversight of Physician Assistants and Nurse Practitioners and others providing delegated services to patients.

  Evaluates patients'/significant others'/staff educational needs and develops and implements appropriate teaching strategies or delegates to appropriate resources.

  Documents in a manner consistent with regulatory standards.

  Communicates with other members of the health care team in order to ensure continuity of care and coordination of services. Acts as a patient/family advocate.

  Participates in ongoing process improvement activities through the development/application of clinical paths, process improvement and other related activities.Actively works with Spectrum Health to meet financial and patient satisfaction goals.

  Qualifications

  Required Bachelor's Degree

  LIC-Physician (MD) - STATE_MI State of Michigan Upon Hire required Or

  LIC-Osteopathic Physician (DO) - STATE_MI State of Michigan Upon Hire required

  CRT-DEA Registration - STATE_MI State of Michigan Upon Hire required

  CRT-Basic Life Support (BLS) - AHA American Heart Association 120 Days required Or

  CRT-Basic Life Support (BLS) - ARC American Red Cross 120 Days required

  CRT-Adv Cardiovascular Life Support (ACLS) - AHA American Heart Association If required by specialty or stress test performed 120 Days required
